Our Home Care Services

Country Carers provides a high standard of care across the local community to people who still live in their own homes but require some additional support. This includes: personal care, domestic & shopping support, transport, overnight & 24-hour care, and palliative care. It enables them to maintain their independence and quality of life. Country Carers recognises and respects the importance of independence of its Service Users.

We encourage and help those under our care to continue to pursue an active life in the community. We offer a complete service that is totally flexible from ad hoc calls to 24-hour live in care. Country Carers offers its services to older people over 65 years and adults between the ages of 18-65 years with learning and/or physical disabilities. Also, for those suffering with dementia, mental health problems, sensory impairment, and terminal illness.

Personal Care

Assistance given with the needs of your daily living – such as bathing, dressing, medication support, continence care and meal preparation.

Domestic & Shopping Support

Support given with household duties – for example, vacuuming, polishing, cleaning, laundry, ironing, washing-up, grocery shopping, pet care and general light household duties.

Transport

Our team can eliminate the inconvenience of taxis by driving you to and from your medical/social appointments and delivering/collecting prescriptions.

Overnight/24-Hour Care

We are fully aware that situations can change. We offer short term overnight care and/or 24-hour care to meet your specific needs and requirements.

Palliative Care (End of Life)

We understand how difficult and distressing it can be to cope with a loved one who is approaching their end of life. Country Carers can offer help and support with day to day tasks so you and your family can make the most of your time together.

Initial Consultation

Please call or email us to arrange your initial chat. Our care manager will visit you for a free, home assessment to discuss your needs and how we would be able to help.

Tailored Care Plan

Following the initial meeting, our care manager will document your needs and devise a support plan that best suits you and will discuss any changes you would like to make.

Meet Your Care Team

We will then introduce our carers to ensure that you are happy with the people who will deliver the service, before they start working for you.

On-Going Support

Throughout your time with us, we believe that continual monitoring of what we do and, more importantly, communication with you, is vital to ensure that you get the service you require.
